(C5-3) Lightning Talk | Better Together: How Collaborative Cross-Sector Partnerships Transform Outcomes for Youth with Complex Needs
Description

This Lightning Talk showcases NEOFACS’ Intensive Services Program (ISP)—a proof-of-concept for intentional cross-sector collaboration uniting child and youth mental health services, child welfare, and key community partners to transform outcomes for Ontario’s most complex youth. 
 The ISP is a live-in care program offering support to our community’s most vulnerable youth, including those facing housing instability, mental health challenges, substance use, or risk of human trafficking. Over 19 months, ISP supported 21 unique clients across 26 placements and 2,394 days of care, with 57% of youth successfully transitioning to supported housing, live-in treatment, or their family home. 
 This collaborative model—built with community partners including adult mental health and substance use services, withdrawal management teams, and youth wellness hubs—proves that when services align expertise instead of maintaining silos, youth can stabilize, engage in treatment, and move toward independent living.
