(F3) Connecting Children and Youth to Mental Health Services through Integrated Care Pathways
Description

Integrated care pathways are evidence-based interventions that guide standardized care and improve inter-provider communication. Such pathways typically include the use of standardized screening tools (e.g., HEADS-ED, RCADS, CRAFFT) to guide assessments and communicate needs. The Knowledge Institute on Child and Youth Mental Health and Addictions has supported the development and implementation of pathways that connect children and youth with community mental health services. These pathways include: 1) MyDAWN, which has been implemented in a primary care setting in South Georgian Bay; 2) an emergency department pathway implemented at Pembroke Regional Hospital; and 3) 1Call1Click.ca, a coordinated access and navigation service in Eastern Ontario. In collaboration with community partners, we will present an overview of each pathway and highlight key data demonstrating pathway uptake in each region.

1. Describe integrated care pathways and understand how they can improve client care. 

2. Demonstrate how integrated care pathways can connect children, youth, and families to mental health care in various clinical settings.
